Just use the PayPal API to setup payments on your site, when you get the OK from paypal via the return code you can proccess the download, create a license key, whatever you need to do.
This is the method I plan to use for Gem Heroes as almost everyone has a paypal account and or a credit card. The cost is 2.9% + $0.30

The release for other countries will have to be planned anyway with a server dedicated to just them and a re-written language (all displayed text is contained in a single file for ease of translation) file for non-english gameplay.
