Software to log/organise improvements/bugs

Hi can anyone recommend some software to log improvements and bugs. They can be two different apps. Sure I could write one in Excell (hmm) or Access (better) but I want something with the correct fields and built in organisation features and ease of use.

This isn't for my games but for the contract work I do on a massive piece of client/server software. When making stuff you don't have time to code all the cool ideas you come up with so you make notes of possible future improvements that are periodically reviewed for adding into a new version (or buried). For example, you need to log the following:

Date improvement thought of
version of software at date
Who thought of it (initials are fine)
Which software module it affects e.g. accounts, customer orders, stock control etc.
Brief one-line description
Detailed description/notes
How cool it is (low/Med/high etc)
How easy is it to make (low/med/high)
Coding time estimate

and possibly some other stuff, but that would pretty much do it nicely. Then the info can be sumamrised and ordered so a manager can make a decision like, show me all the cool customer order tweaks that are easy to make, add up the estimated time, OK, I authorise nos 1,2,5 etc, get on and code them

Bug logging is more common I think, so I just need some recommended apps.
